Forms LOC020, LOC021 and LOC025: Change your name by deed poll

If you're 18 or over, use these forms to put your new name on public record by ‘enrolling’ your deed poll at the Royal Courts of Justice. Includes guidance note LOC019.



LOC019 is guidance for filling in:

  • Form LOC020 (the change of name deed for an adult, also known as the ‘deed poll’) 

  • Form LOC021 (the statutory declaration for an adult name change
  • Form LOC025 (the Notice for the London Gazette for an adult name change)
