Online forms

To make a single CHAPS payment to cover a number of Self Assessment references (called a ‘multiple’ or ‘composite’ payment) please contact your HMRC Accounts Office. Your Self Assessment tax bill tells you which office your account is linked to.

Your message using these forms are secure. If HMRC reply by email the information is not secure. If they need to contact you about anything confidential, they will reply by post or telephone.

If you can’t make a payment using any other method you can send a cheque with your payslip to HMRC. Make your cheque payable to “HM Revenue and Customs only” followed by your Self Assessment Unique Taxpayer Reference. Don’t fold payslips or cheques or fasten them together.
You do not need to include a street name or PO box in the address and should allow at least 3 working days for your payments to reach them.
If you don’t have a payslip you can print off a replacement payment slip.
