You can submit the form any time in the 48 hours before you arrive in the UK.
Provide a negative coronavirus (COVID-19) test to travel to the UK
You must also provide proof of a negative coronavirus test before you leave for the UK. The test must have been taken in the 3 days before your departure. The proof must be in English, French or Spanish.
You must provide the proof to the company you’re travelling with - for example, the airline, ferry company or train operator.

You’ll be committing a criminal offence if you do not have proof of a negative test or you have not completed the passenger locator form. You may be fined. You may not be allowed to board. If you are not British or Irish, you may not be allowed to enter the UK.
Before you start
To complete the passenger locator form you need:
- your passport details
- your travel details, including times and dates
- the address where you will stay in the UK (if applicable)
- a booking reference number and the name of the test provider, if you’re using Test to Release to find out if you can end self-isolation early
- booking reference numbers for the 2 coronavirus tests you must get after arriving in the UK
- a booking reference number confirming your quarantine hotel booking if you need one

You can include multiple journeys in your form if you’ll enter the UK more than once in a 48-hour period.
It is a criminal offence to provide false or deliberately misleading information when filling out your Passenger Locator Form. You could be fined, imprisoned or both if you do not provide accurate details about the countries you have visited in the 10 days before you arrived in the UK.

You can use this service to:
- fill in and submit a passenger locator form before you arrive in the UK
- set up an account to save your details if you travel frequently
- sign in to your account to edit and submit an unfinished form
After you complete the form you will receive a QR code via email. You can print this out or show it on your phone as proof of completion.
What you need to know
The information you give will be used to contact you if someone you travelled with develops coronavirus symptoms. It may also be used to check that you are either:
- self-isolating in the place you’re staying in
- quarantining in a hotel
Your information will be kept for 42 days then deleted, unless it’s needed for related work.
If you create an account, the information you’ve saved will be kept for up to 6 months from the date you last signed in. Your account and all the information stored in it will then be deleted.
If you’re travelling with other people
Each adult must complete their own form.
You can include someone under 18 years old who is travelling with you on your form, if you are staying together at the same UK address.
If you’re entering Scotland, they must be under 16.
When you do not need to complete the form
You do not need to complete the form if you’re travelling from one of the following places, and you were there for 10 days or more:
- the Channel Islands
- the Isle of Man
You do not have to complete the form if your job means you are exempt.
If you need to update your details
If any of your information changes after you have submitted your form, for example your travel details or contact information, you must complete a new form.
