Health declaration form for childminders and people who work in childcare or children's social care.

Introduction
You will need to complete the health declaration form before you register with Ofsted to look after or care for children. This form used to be known as the health declaration booklet.
Please complete this form if:
- you’re applying to register as a childminder on the Early Years Register
- you’re registering as the manager of childcare on domestic premises
- you’re applying to register as a social care establishment, agency or manager
- Ofsted has asked you to complete it because we need more information (for example, if you live with a childminder or if you own a nursery)

Social care applications
It may be difficult to get your health declaration form returned promptly by your GP at this time. If your GP is unable to return your health declaration form within 2 weeks, you can complete our additional health self-declaration form to submit with your application.
If you complete this health self-declaration form correctly, we will accept your application and will review the GP’s form when it arrives. We will not be able to register you without the GP’s form, so you still need to send this to us later.

Completing the health declaration form: early years and social care
All applicants must complete this form.

Fill in section A, the relevant part of section B and the declaration.

Contact your GP to fill in section C. GPs may charge a fee for doing this.

Either scan or photograph the document and send with your unique reference number (URN) to enquiries@ofsted.gov.uk.
Completing the additional social care health self-declaration form
Only applicants for children’s social care may complete this form in addition to the health declaration form, when your GP is unable to return your health declaration form within 2 weeks.
You must still send us the health declaration form when your GP has completed it.

Your information
You can find information on how Ofsted handles personal information in our personal information charter.
This information remains valid for 6 months from the date of your GP’s signature (health declaration form) and/or your signature (health self-declaration form). Please send this to Ofsted without delay.
It is an offence to make a statement that you know is false or misleading as part of a registration application. Please answer this form truthfully. We will not necessarily refuse your registration based on current or previous health problems.
