Health declaration form for childminders and people who work in childcare or children's social care.
You will need to complete the health declaration form before you register with Ofsted to look after or care for children. This form used to be known as the health declaration booklet.
Please complete this form if:
- you’re applying to register as a childminder on the Early Years Register
- you’re registering as the manager of childcare on domestic premises
- you’re applying to register as a social care establishment, agency or manager
- Ofsted has asked you to complete it because we need more information (for example, if you live with a childminder or if you own a nursery)
Social care applications
If you are applying to register as a social care establishment, agency or manager from 5 May 2020, you can choose to complete a health self-declaration form in addition to the health declaration form.
This is a temporary measure to allow you to register during the COVID-19 (coronavirus) pandemic, when it may be more difficult for your GP to complete and return your health declaration form.
You must still complete the health declaration form and send it to your GP.
If you need any help completing the form, please telephone us on 0300 123 1231 or email email@example.com.
