This form is replacing forms ESA50 and UC50. This new WCA50 form will be issued by our Assessment Providers from 24 November 2025.
If you have already filled in a ESA50 or UC50 form, this will be accepted by the Health Assessment Advisory Service.
Fill in this form only if you’ve been asked to do a Work Capability Assessment.

If you’re using a mobile phone or tablet
