COVID-19 vaccination programme

2020 has proved to be a very challenging year for our population and for the national immunisation programme. The teams delivering immunisation in general practice, pharmacies, midwifery units and schools have all contributed to an amazing effort to ensure that that this incredibly successful public health programme continued to be offered despite the impact of this unprecedented situation.

Thank you to everybody who has continued to work so hard to ensure the delivery of this important service – wherever and however it has been achieved.

Now a new challenge faces us – to deliver the COVID-19 immunisation programme. This is a great credit to all those who have been involved from the development of the vaccine through to first offering this intervention to those who are most at risk and those frontline social and health care workers offering them care.

Extensive planning has been underway to enable us to start the programme and offer this important protection, starting with our older population and frontline social care workers and healthcare workers.

At the time of publishing 521,594 COVID-19 vaccinations have been given and it is very reassuring to see all the examples of older adults in all four nations having their first vaccination.

This is a huge task but at the same time a great privilege to be involved in this offer of protection to our population. As we end this very difficult year, we look forward to the COVID-19 vaccination programme being rolled out to the eligible groups as quickly as possible and it is our fervent hope that everyone offered takes up this vaccine and eventually help to protect the population from this infection.

There is a new chapter in Immunisation against infectious disease, also known as the Green Book, a vital guide for public health professionals administering vaccines in the UK.

The new COVID-19 chapter includes information on:

  • COVID-19 vaccines
  • recommendations for the use of the vaccine
  • guidance on the storage, dosing and schedule of the vaccines
  • provisional priority groups recommended by the Joint Committee on Vaccination and Immunisation (JCVI), to help the NHS with its deployment planning
  • advice on vaccination of high risk groups
  • guidance on potential adverse effects, which current evidence suggests are mild and short-term

Information with respect to the AstraZeneca vaccine is provisional subject to the Medicines and Healthcare products Regulatory Agency (MHRA) approval to supply.

This chapter was originally published on 27 November and was revised on 5 December in relation to updates from the newly licensed Pfizer BioNTech vaccine and then revised again and republished on the 11 December to include further clarification on severe allergies.

The Green Book how vaccines work chapter 1

The Green Book, immunity and how vaccines work, chapter 1 has been revised to include information on the newest types of vaccines which use the pathogen’s genetic code as the vaccine; this then exploits the host cell’s apparatus (including enzymes and ribosomes) to translate the proteins that then act as an intracellular antigen and stimulate the immune response (van Riel and de Wit, 2020).

Priority risk groups

For full information on priority risk groups please see Green Book chapter 14a

  • residents in a care home for older adults
  • staff working in care homes for older adults
  • all those 80 years of age and over
  • frontline healthcare and social care workers

Recommendations by staff groups

The objective of occupational immunisation of health and social care staff is to protect workers at high risk of exposure who provide care to vulnerable individuals.

Although there is yet no evidence on whether vaccination leads to a reduction in transmission, a small effect may have major additional benefit for staff who could expose multiple vulnerable patients and other staff members. Potential exposure to COVID-19, and therefore the priority for vaccination, may vary from workplace to workplace. Guidance on COVID-19 immunisation that may be appropriate follows.

Social care workers

This would include:

  • those working in long-stay residential and nursing care homes or other long-stay care facilities where rapid spread is likely to follow introduction of infection and cause high morbidity and mortality
  • social care staff directly involved in the care of their patients or clients
  • others involved directly in delivering social care such that they and vulnerable patients/clients are at increased risk of exposure

Young people age 16 to 18 years, who are employed in, studying or in training for health and social care work should be offered vaccination alongside their colleagues if a suitable vaccine is available. Younger people who are taking part in health and social care work as volunteers, interns or for the purposes of work experience, should make all efforts to avoid exposure to infection; vaccination would not normally be required.

Healthcare staff

All frontline healthcare staff who are eligible for seasonal influenza vaccination should be offered COVID-19 vaccine.

This includes staff who have frequent face-to-face clinical contact with patients and who are directly involved in patient care in either secondary or primary care/community settings. This includes doctors, dentists, midwives and nurses, paramedics and ambulance drivers, pharmacists, optometrists, occupational therapists, physiotherapists and radiographers.

It should also include those working in independent, voluntary and non-standard healthcare settings such as hospices, and community-based mental health or addiction services. Temporary staff, including those working in the COVID-19 vaccination programme, students, trainees and volunteers who are working with patients must also be included. Non-clinical staff in secondary or primary care/community healthcare settings. This includes non-clinical ancillary staff who may have social contact with patients but are not directly involved in patient care. This group includes receptionists, ward clerks, porters and cleaners.

Laboratory and pathology staff

Hospital-based laboratory and mortuary staff who frequently handle SARSCoV-2 or collect or handle potentially infected specimens, including respiratory, gastrointestinal and blood specimens should be eligible as they may also have social contact with patients. This may also include cleaners, porters, secretaries and receptionists in laboratories.

Frontline funeral operatives and mortuary technicians/embalmers are both at risk of exposure and likely to spend a considerable amount of time in care homes and hospital settings where they may also expose multiple patients. Staff working in nonhospital-based laboratory and those academic or commercial research laboratories who handle clinical specimens or potentially infected samples will be able to use effective protective equipment in their work and should be at low risk of exposure.

Arrival of publications

We are now working with NHSE/I to finalise the plan to immediately supply all the Trusts and PCNs directly with a delivery of publications. Individual Trusts, Mass Vaccination centres and PCNs do not need to place orders directly with Health Publications at this stage as we have a publications pathway in place.

Due to the short period between vaccine approval and roll-out we are doing all we can to get supplies to you. In the interim, please use the links to download and print resources. Print ready files are also available from the Health Publications website.

Translations and alternative formats

The process of translating leaflets has now commenced and we will be producing the alternative formats that will include the following languages: Arabic, Albanian, Chinese, Bengali, French, Farsi, Kurdish, Gujarati, Hindi, Polish, Panjabi, Nepalese, Romanian, Turkish, Tagalog, Spanish, Somali, Ukranian and Urdu.

We will then go on to provide short audio versions for people with low literacy in English and their native language.

Easy Read leaflets

Easy Read versions of the guide for adults, what to expect after your vaccination and the guide for pregnant or childbearing age leaflets will be available soon for people with a learning disability and their carers or for individuals with low literacy.

British Sign Language videos

BSL video versions of the guide for adults, what to expect after your vaccination and the guide for pregnant or childbearing age leaflets are available to download or view on the Health Publications website.

Explainer videos

NHS England Learning Disability and Autism Programme have produced a new general immunisations video for people with a learning disability and their carers. A set of short social media clips are available to download on the Health Publications website to promote the video.

Quick video guides to the COVID-19 programme are being produced to help answer questions on COVID-19 vaccination. These will be available to download or view on Public Health England YouTube.

Accessibility is at the forefront of our provision. We will publish the above-mentioned products as soon as possible.

Printing and downloading resources

For any deliveries required for the last week of December we recommend placing your orders via the usual route or on the Health Publications website and the last date of dispatch is the 22 December. Normal deliveries will resume on the 4 January.

There is a good level of stock of all the 23 portfolio items and capacity to supply any further deliveries requested from the 19 to 21 December and all parties have been advised to ensure they have enough stock until the 4 January.

Subject to the decision of the MHRA and the licensing of the second vaccine, it is possible that we may have to make revisions to the core materials including the training guidance and slide sets, and the following leaflets are most vulnerable to these changes:

  • COVID-19 vaccination – a guide for adults
  • COVID-19 vaccination – a guide for women of childbearing age, pregnant or breastfeeding
  • what to expect after your vaccination leaflet

Leaflets, posters, flyers, consent forms and invitation letters can all be downloaded and printed locally. This is an important interim measure as we await new printed supplies.

We will also send out notification if any local stores of leaflets that are not suitable to accompany the second vaccines, they are to be destroyed.

In the event of deliveries of the new vaccine to new PCN or vaccine delivery locations we recommend printing locally immediately as artwork is made available as we are unable to deliver in these time frames. We are working hard to ensure deliveries as soon as possible.

Letter templates are available as ODT and Word formats on the .GOV website. Print ready files with crop and bleed marks for commercial printing are also available on the Health Publication websites at the product codes next to the product names in the resources section above.

Ordering clinical publications and resources

Record cards are supplied with the vaccine at a ratio of 1 dose of vaccine to 1 record card. You do not need to order record cards as they are supplied with the vaccine unless you have additional vaccine quantities, and require 200 more due to vaccine supply. In this case please order 4 packets of 50.

Please only order supporting resources if you have not received a delivery or do not already have a scheduled delivery of leaflets and resources from Public Health England via NSE/I. This will help ensure continuity of supplies. Initially it may be necessary to print resources locally as deliveries will commence as soon as possible but are delayed due to the short period between vaccine approval and roll-out. Supply will rapidly stabalise, and is currently matched to vaccine supply. This is why it is very important for sites not yet in receipt of immediate deliveries, should not place orders until this is established.

All stock is subject to significant changes and we cannot produce huge volumes of waste during this evolving process.

You will note that if you view any of these items on the system now, they show as out of stock, please ignore this, as we are waiting for stock to arrive. You can still place an order now and it will be fulfilled as soon as stock is available.

Please do not order more stock than you have vaccines for

More stock will be regularly available and a continuous supply of additional stock is essential. All resources may be subject to revisions and updates. Stock piling is not required and should be avoided. It is important to manage leaflet versions carefully and dispose of old versions as soon as the new ones are published and made available.

If you’re on Facebook, you can add our “I’ve had my COVID vaccination” sticker to your profile picture to help encourage others to get their vaccine. There are 3 versions – one for NHS staff and one for social care workers and one with no logo.

Colleagues from Devolved Administrations please refer to guidance from your respective health departments for arrangements in Scotland, Wales and Northern Ireland.

How to place your order

All account holders will have access to the vaccine ordering page where you can order any number of packs up to your maximum pack allocation in one go or split over a number of orders each week.

Allocation of packs available to each site in England has been determined by NHS England and queries concerning your allocation should be directed to england.spocskh@nhs.net and england.covidvaccs@nhs.net.

Allocations will be updated in line with available inbound vaccine deliveries to PHE:

  • sites in England will receive a notification email from ImmForm confirming when their allocation has been increased and they can proceed to ordering
  • sites in Scotland will receive a notification email from ImmForm asking them to go to their ImmForm allocation report

For the Pfizer/BioNTech vaccine, each pack of vaccine ordered (975 doses), will be automatically linked to 5 associated products which will arrive with the vaccine pack:

  • 0.9% sodium chloride diluent for injection (200 ampoules, box presentation will vary)
  • 2ml syringe/needle for the dilution step (2 boxes of 100 units)
  • “Regulation 174 Information for UK recipients” leaflet (box of 1000)
  • PHE-issued vaccination record card (box of 1000)
  • 1ml combined needles and syringes for administration (12 boxes of 100 units)

Extra vaccination record cards can be ordered should you require. These are available in packs of 50. Please only order what you need.

The electronic version of the Regulation 174 leaflet for UK recipients leaflet is available on the MHRA website. Please print the desired volume locally as required.

All the vaccine associated products will be delivered at ambient temperature. These should be stored in a dry environment and kept away from sunlight.

Needles for administration of vaccine to morbidly obese individuals are a separate item and should be ordered in addition if required. These can be ordered independently of vaccine.

The rolls of yellow thawing labels (previously provided via separate arrangements between the NHS and Pfizer) are now available to order via ImmForm, and can be ordered as a separate item in a roll of 100 labels which is sufficient for 100 packs of vaccine (each pack containing 975 doses).

For this reason orders are currently capped at 1 roll of labels. Customers that specifically require an additional roll of labels are asked to contact the ImmForm Helpdesk for assistance at helpdesk@immform.org.uk or 0207 183 8580.

When to expect deliveries

Vaccine and associated products are expected to be delivered on a next day delivery schedule Monday to Saturday for orders placed before 11:55.

  • orders placed before 11:55am Monday to Friday will receive next day delivery, Tuesday to Saturday respectively
  • orders placed after 11:55 Friday and before 11:55 Saturday will be delivered Monday
  • orders placed after 11:55 Saturday and before 11:55 Monday will be delivered Tuesday

There are exceptions to these schedules for some customers:

  • Scottish customers in Shetland, Orkney, Stornoway, Fort William, Inverness and Wick will use scheduled delivery days agreed for the ordering of COVID-19 vaccines
  • Jersey & Guernsey will receive 48-hour delivery but no deliveries Sunday or Monday

Separate delivery schedules will be in place over the Christmas holiday period. Details for these are below.

Important information on vaccine handling

The Pfizer/BioNTech vaccine will be delivered at ultra-low temperature, between -80°C and -60°C. Vaccine packs will be shipped in isothermic boxes validated for up to 84 hours.

The isothermic box will contain a quantity of dry ice which will need to be disposed of following local protocols. The approximate weight of the dry ice will be a maximum of around 10kg.

Each external carboard box will have the address label, the class 9 hazard warning label and will be sealed with tamper proof tape.

The time and date of expiry of the validated storage timeframe will be clearly marked on the cardboard box, together with a Movianto customer helpline.

The vaccine should be unpacked and transferred to a suitable ultra-low temperature freezer as quickly as possible to ensure product is continued to be stored between -80°C and -60°C.

One box from each order will have the despatch note that has details of product, quantity, batch and expiry (in the Documents Enclosed wallet).
