This guidance provides a practical guide to completing the pre-exposure request form for individuals who may have an increased risk of rabies exposure and therefore are recommended to receive rabies pre-exposure vaccination paid for by the Department of Health.

Risk assessment and provision of the vaccine is through PHE’s Rabies and Immunoglobulin Service (RIgS) at PHE National Infection Service, Colindale.
