Print off and fill in the Sure Start Maternity Grant (SF100) claim form.
There’s a different form and postal address if you live in Northern Ireland.
You can claim from 11 weeks before the week your baby is due. The latest you can claim is 3 months after your baby is born.
If you’re becoming responsible for a child, eg through adoption, you must claim within 3 months of this happening (eg the child being placed with you).
Where to send your form
You can either:
- take the form to your local Jobcentre Plus
- post it to “Freepost DWP SSMG” (you don’t need to write anything else on the envelope)

What you need to know
A health professional (eg doctor or midwife) must also sign your claim form.
You’ll get a letter telling you if your claim was successful.
Appeal a decision
You can appeal to the Social Security and Child Support Tribunal if you disagree with a decision. You must usually ask for ‘mandatory reconsideration’ before you appeal.
