Forms and guidance to register a lasting power of attorney made using forms LP PA, LP PW, LPA114 or LPA117.
Use these documents to:
- apply to register a lasting power of attorney (LPA) made on forms LP PA or LP PW, which must have been signed and dated correctly before 1 April 2011
- apply to register an LPA made on forms LPA114 or LPA117, which must have been signed and dated correctly before 1 January 2016
- let people know about your application
- apply to pay a reduced fee if the donor has a low income
If your LPA form LP PA, LP PW, LPA114 or LPA117 hasn’t been signed and dated correctly by the dates shown above, you need to create a new LPA. It’s easiest to create one using the digital LPA service, or you can use the new LPA forms, LP1F or LP1H.
The guide (LP13) explains how to apply to register your LPA using the old forms. You can download the guide and forms individually or as a complete pack of documents.
If you are downloading a complete pack of documents, look on your computer’s desktop or downloads folder for a file with ‘LPA’ in the title and ending with ‘.zip’. Double click on this file and your computer will unpack it as a folder containing all the documents you need.
You can fill in these forms on your computer and then print them out, sign them and send them to the Office of the Public Guardian at the address shown on the form.
If you made your LPA online using the digital LPA service, you need to use the digital LPA service to register it.
