You can download:
- a complete pack of documents for each type of LPA
- individual forms and guidance
If you’re downloading a complete pack of documents, look on your computer’s desktop or in the downloads folder for a file with ‘LPA’ in the title and ending with ‘.zip’. Double click on this file and your computer will unpack a folder containing all the documents you need.
You can fill in these forms on your computer and then print them out and send them to the Office of the Public Guardian at the address shown on the form.
If you’ve downloaded an LPA form (LP1F or LP1H) from this page, you use the same form both to make your LPA and to register your LPA.
However, you need to download a separate registration form if your LPA was made using one of the following forms:
You can only register forms LPA114, LPA117, LPA PW or LPA PA if they were signed and dated correctly by 1 January 2016. If they weren’t signed and dated correctly by this date, you’ll need to create a new LPA by using either the online service or the forms on this page.
