Paper forms and guidance to create and register a lasting power of attorney (LPA).
These paper forms and guidance leaflets are for people who want to make a lasting power of attorney (LPA) for either:
- financial decisions
- decisions about health and personal care
You can download a complete pack of documents for each type of LPA or individual forms and guidance. You use the same form both to make and to register your LPA.
If you are downloading a complete pack of documents, look on your computer’s desktop or in the downloads folder for a file with ‘LPA’ in the title and ending with ‘.zip’. Double click on this file and your computer will unpack a folder containing all the documents you need. You can fill in these forms on your computer and then print them out and send them to the Office of the Public Guardian at the address shown on the form.
Alternatively you can make a lasting power of attorney online – it’s easier.
If your LPA was made on forms LPA114, LPA117, LPA PW or LPA PA, you need to download a separate registration form. However, you can only register forms LPA114, LPA117, LPA PW or LPA PA if they were signed and dated correctly by 1 January 2016. If they weren’t signed and dated correctly by this date, you’ll need to create a new LPA using the digital service or by downloading one of the forms on this page.
