Details

The analysis was carried out by the UK Health Security Agency (UKHSA) and has been reviewed by the Patient Safety in Radiotherapy Steering Group.

Reports 3 to 6 covering the period December 2011 to December 2019 were published by Public Health England (PHE).

Report 2 covering the period December 2009 to November 2011 was published by the Health Protection Agency (HPA).

Reports prior to this were published by the HPA and are available via their archived web pages.
