Details

Before you fill in these forms read the guidance on transporting assistance dogs by sea to check if you must carry assistance dogs with their owners.

If you want to agree an MOU for a new route, fill in both forms and send them to the Animal and Plant Health Agency (APHA).

If you want to change an existing MOU, fill in the addendum to the MOU (PT41) and send it to APHA.

APHA will confirm the MOU in writing and add your route to the list of carriers registered to transport assistance dogs.
