Use this form to notify the Driver and Vehicle Standards Agency (DVSA) if you make changes to the specification of a PSV. There is no fee for this.

You don’t need to provide notification of alterations for the normal like-for-like replacement of worn, defective or damaged parts on a PSV.

Your application will be delayed if you don’t fill in the form correctly.
