These documents contain:
- a laboratory request form for health protection teams (HPTs) to fill in and provide with the measles, mumps and rubella (MMR) oral fluid kit to parents to post
- a template letter for HPTs to send to patients or parents whose children have suspected cases of MMR
- pictogram on how to take an oral fluid swab
- instructions for HPT staff how to prepare to send, procure and find the results from oral fluid kits
Health Protection Teams should:
- add patient details and local signature and contact information to the letter
- complete as much information as they can on the laboratory request form
- ensure all the appropriate documentation is included with the oral fluid swabs prior to sending them out to all patients with suspected measles, mumps or rubella
