Use these forms to:
- inform UKVI of a relationship breakdown
- give UKVI permission to use information about your estranged partner
You should only include one of these forms in your letter to the Home Office.
You should use the public statement if you don’t want the Home Office to tell your ex-partner any details from your letter.
You should use the consent form if you’re happy for the Home Office to tell your ex-partner details from your letter
