Use this form to ask the court to determine someone's parentage and to give your summary of why you think your application is necessary.

When you apply, include a copy of the birth certificate of the person whose parentage is in question (unless the court has directed otherwise).
Check court and tribunal fees and find out if you can get help with fees.
