You will need to fill in the Timesheet and Supporting Evidence spreadsheet along with this form and send both documents to us for approval.
The Timesheet and Supporting Evidence spreadsheet should have been sent to you by email. If you do not have it, email ruralpayments@defra.gov.uk or call us on 03000 200 301 and we will send you a copy.
