Details

  • You must be a member of a Facilitation fund group to request Facilitator endorsement.
  • Your Facilitator will need to fill in the form on your behalf and return it to you. We may request this form from you as supporting evidence for your CS application.
  • Your Facilitator must keep a copy of the form for their records if they want to claim payment for time spent endorsing your CS application.
