Details

A single application made using form AFS1 covers an entire legal entity, including all departments, branches and sub-offices.

Submit your application

Remember to sign the form (on page 5), we cannot process your application unless this is done. We cannot accept scanned images of signatures or electronic signatures.

Fill in, sign and return form AFS1 (to the address on the form) with any other relevant documents listed in Apply for Business e-services:

  • variable direct debit application form and mandate
  • Network Access Agreement
  • Memorandum of Understanding for lenders
  • Additional administrator details continuation sheet AFS4
  • Deputy responsible person details AFS5
