This data report presents data against each of the 15 screening standards for screening activity related to abdominal aortic aneurysm (AAA) screening.
The data source used for this report is AAA SMaRT (screening management and referral tracking).
The NHS AAA screening programme aims to reduce aneurysm-related mortality through early detection, appropriate monitoring and treatment. The NHS AAA screening programme invites men for ultrasound screening during the year (1 April to 31 March) in which they turn 65. Men over 65, who have not previously been screened can self-refer. The programme offers annual or quarterly surveillance for all men who on initial scan are found to have a small or medium aneurysm respectively. Men found to have a large aneurysm are referred into the care of specialised vascular services for potential intervention.
National AAA standards provide a defined set of measures that providers must meet to make sure services are safe and effective. The NHS AAA screening programme published a set of revised standards in April 2020.
This report is an official statistic. Official statistics are an essential public asset. They provide a window on society, the economy, and on the work and performance of government. They are fundamental to the judgements and decisions made by the public, by government, and by a large range of other organisations.
Figures are rounded to one decimal place for presentation purposes. Therefore, some rounded values may appear to meet the threshold when the unrounded value does not.
1. AAA Standards
There are 15 national AAA standards included in this report, which are:
* AAA-S01: completeness of offer (initial screen)
* AAA-S02: completeness of offer (annual surveillance scan)
* AAA-S03: completeness of offer (quarterly surveillance scan)
* AAA-S04: coverage (initial screen)
* AAA-S05: coverage (annual surveillance scan)
* AAA-S06: coverage (quarterly surveillance scan)
* AAA-S07: coverage of the initial screen in men living in deciles 1 to 3
* AAA-S08: uptake (initial screen)
* AAA-S09: uptake (annual surveillance scan)
* AAA-S10: uptake (quarterly surveillance scan)
* AAA-S11: non-visualised screens
* AAA-S12: internal quality assurance
* AAA-S13: nurse assessments
* AAA-S14: men seen at specialist assessments within 2 weeks
* AAA-S15: men operated on within 8 weeks
Standards data are published annually. Standard AAA-S04, AAA-S05 and AAA-S06 are also key performance indicators (KPIs), which are published quarterly.
Two thresholds, the acceptable threshold and achievable threshold are specified for each standard.

Data are presented by financial year (1 April 2025 to 31 March 2026) unless stated otherwise.
2. AAA-S01: completeness of offer (initial screen)
There were 353,187 men aged 65 eligible for AAA screening between 1 April 2025 and 31 March 2026; 99.7% of those eligible men were offered an initial screen within the screening year plus 2 months. The reasons for the remaining men not being invited include:
* offered after 31 May 2026
* incorrect contact details
* deferring screening
* opting out of the screening programme
* not offered

* 12 out of the 37 screening providers also reached the achievable threshold of ≥ 99.9%
* Shropshire Telford and Wrekin AAA Screening Cohort (98.1%) had the lowest performance but still met the acceptable threshold

The offer of the initial screen to men in each cohort year was stable between 2013/14 and 2019/20 (see previous years’ reports for data prior to 2017/18). Following the sharp drop in 2020/21 performance due to the impact of the COVID-19 pandemic, performance increased year on year. For the last 3 years, 2023/24 (99.8%), 2024/25 (99.8%) and 2025/26 (99.7%), performance has returned to pre-pandemic levels . The AAA screening programme is meeting the acceptable threshold and just below the achievable threshold.
3. AAA-S02: completeness of offer (annual surveillance scan)
Between 1 April 2025 and 31 March 2026 there were 11,779 annual surveillance appointments due and 11,623 had an offer within 6 weeks either side of their due date (98.7%).

* Lancashire and South Cumbria AAA Screening Cohort (95.9%) had the lowest performance but still met the acceptable threshold
Nationally the proportion of annual surveillance appointments due where an appointment was offered 6 weeks either side of the due date increased from 98.4% in 2024/25 to 98.7% in 2025/26, meeting the acceptable threshold.
4. AAA-S03: completeness of offer (quarterly surveillance scan)
Between 1 April 2025 and 31 March 2026 there were 11,301 quarterly surveillance appointments due and 11,158 had an offer within 4 weeks either side of their due date (98.7%).

5. AAA-S04: coverage (initial screen)
Of the 353,187 men aged 65 eligible for AAA screening between 1 April 2025 and 31 March 2026; 82.2% of those eligible men were conclusively tested within the screening year plus 2 months.

Following the decrease in performance in 2020/21, due to the impact of the COVID-19 pandemic, performance has increased to 82.2% in 2024/25 and maintained 82.2% in 2025/26, meeting the acceptable threshold but not meeting the achievable threshold.
6. AAA-S05: coverage (annual surveillance scan)
Of the 11,779 annual surveillance appointments due between 1 April 2025 to 31 March 2026, 91.9% had a conclusive test within 6 weeks either side of the due date.

Nationally the proportion of annual surveillance appointments due that had a conclusive test 6 weeks either side of the due date decreased by from 92.3% in 2024/25 to 91.9% in 2025/26, meeting the acceptable threshold.
7. AAA-S06: coverage (quarterly surveillance scan)
Of the 11,301 quarterly surveillance appointments due between 1 April 2025 to 31 March 2026, 92.9% had a conclusive test within 4 weeks either side of the due date.

Nationally for 2025/26 the percentage of men in the eligible cohort who were tested and who lived in a lower super output area (LSOA) classed as decile 1 to 3 in the English indices of deprivation (IoD) 2019 was 75.4%. This contrasts with 82.2% for the overall cohort.

Men living in more deprived areas are less likely to attend for screening but are more likely to have an aneurysm. It is important for providers to engage with men living in more deprived areas to make sure they can make a personal informed choice.
This standard should be reviewed alongside standard 4. It was first introduced in April 2020, however performance during 2020/2021 was impacted by the COVID-19 pandemic. Performance has increased year on year and this is the second consecutive year that the acceptable threshold has been met nationally.

14. AAA-S13: nurse assessments
Of the 2,795 men who had a small or medium aneurysm detected (aorta ≥ 3.0 to < 5.5cm) at initial screen and the number of men who had a medium aneurysm detected (aorta ≥ 4.5 to < 5.5cm) at the annual surveillance scan, 75.6% had a nurse assessment within 12 weeks of their conclusive scan in 2025/26.

Nationally the proportion of men eligible for a nurse assessment who attended an appointment decreased from 2024/25 (79.9%) to 2025/26 (75.6%), meeting the acceptable threshold but below the achievable threshold.
15. AAA-S14: men seen at specialist assessments within 2 weeks
Of the 869 men with an aorta ≥ 5.5cm or men with an aorta ≥ 4.0cm that has grown ≥ 1cm in one year, 91.8% were seen by a vascular surgeon within 2 weeks of their last conclusive scan in 2025/26.

Of the 685 men with an aorta of ≥ 5.5cm or men with an aorta ≥ 4.0cm that has grown ≥ 1cm in one year, deemed fit for intervention and not declining 29.6% were operated on by a vascular surgeon within 8 weeks.

The percentage of men operated on within 8 weeks fell to 38.5% in 2019/20 and to a low of 24.1% in 2020/21 during the COVID-19 pandemic. It was above 30% for 3 years; 2022/23 (30.1%), 2023/24 (31.6%) and 2024/25 (31.7%), but has dropped to 29.6% in 2025/26, considerably below the acceptable threshold.
17. Outcomes
The number of fatal and non-fatal AAA ruptures presented are only those reported to the screening provider and will only be for those men who have been screened. This number may, therefore, be an underestimate of the true number of AAA ruptures.
There were 31 AAA ruptures reported during the screening year, 24 of which were fatal. 11 of the 24 deaths were in men on surveillance. 13 of the 24 deaths were in men who had been referred for surgery.
The 11 fatal AAA ruptures in men on surveillance represented 2.1% of the 532 deaths in men on surveillance.
For the men referred to surgery, 5 had been turned down or had declined the operation and 8 died while waiting for surgery. Of the 8 who died while waiting for surgery, 6 died after waiting more than 8 weeks for surgery.
In the previous year, 2024/25, 6 men died while waiting for surgery, 5 of whom died after waiting more than 8 weeks from their referral.
