Evidence from the Ombudsman’s casework has shown repeated failings in the way X-rays and scans are reported on and followed up across NHS services.
These issues are not limited to radiologists, radiographers, or imaging services, but relate to the whole system. The findings have significant implications for all clinical professionals responsible for requesting X-rays and scans, and acting on imaging results.
This report provides recommendations to support the whole system to improve.
