This animal transport certificate and contingency plan should be completed for journeys over 8 hours. It applies to the transport of any vertebrate animals other than cattle, sheep , goats, pigs and unregistered domestic equidae.
Animal transporters can use this form when a journey log is not required.
This form (or the required information if kept in another format) must be kept for 6 months after completing the journey.
