This publication was withdrawn on 26 May 2020

The E28 form is no longer in use. It was implemented at the start of the pandemic but business as usual arrangements for submitting samples to local laboratories now apply. For further information see laboratory guidance at COVID-19: laboratory investigations and sample requirements for diagnosis
