Use the IVA application service to send your completed application form.
This form is used to apply for a replacement IVA certificate but if you find the original certificate then you must return it to the DVSA.
Please read the notes on the form before completing your application and send your completed form to the test station where your vehicle was initially inspected.

Please be aware that you may be required to present your vehicle for inspection in order for DVSA to verify the details.
