Annex D: international engagement

The following are operational recommendations for the UK from international health system experts.

Effective communication and messaging

Attendees agreed that messaging about health system changes should be simple and resonate with the public. Some experts suggested government regularly communicate with the public and key stakeholders to share successes and maintain public engagement with the plan throughout the 10 years. Several attendees described how effective messaging can be used to encourage individual responsibility for health and inform the public of the role of the NHS in supporting this, promoting a shift from treatment to prevention. 

Leadership and involving hospitals and health workforce

Experts emphasised that clinical and political leaders need to work together. They highlighted that involving key stakeholders is essential to build the trust needed for successful reforms. Experts also pointed out the significant role of hospitals, particularly in training future health and care workers. They stressed the importance of addressing workforce burnout and trust issues and preparing staff for digital solutions through new training programs that include digital skills.

Funding reform and flexible funding

There was a consensus that health system reform needs changes in both structure and financing, and that these should happen together. Experts supported flexible funding to encourage local innovation and place-based initiatives. Several contributors shared their experiences with using flexible budgets in neighbourhood health centres to allocate small grants for local needs through participatory budgeting. Participatory budgeting means involving the local community in deciding how to spend part of the budget. Experts emphasised that this approach builds trust and ownership among the local community and that targeted investment can help cover transition costs. Examples from Chile, Zambia and Australia were mentioned.

Community governance

Experts strongly supported involving the community in health planning and decision-making to make sure services meet local needs. Many contributors emphasised that locally-led governance, with strong community oversight and connections to regional and national policymakers, improves accountability and service delivery, particularly when embedded in a learning system approach. They discussed examples from Ontario in Canada, Zimbabwe and Kerala in India.

Leveraging data and technology

Experts agreed that health information systems, data and AI are crucial for reforming health systems. They highlighted how digital health records can make care processes smoother and improve patient management. One attendee shared an example from Coimbra, Portugal’s integrated health delivery system, where better digital pathways and a phone triage system for urgent care have improved integrated care. There was also a discussion on the advantages of health systems owning the technology to control the data. A handful of experts suggested using open-source solutions for quickly developing integrated systems. Others proposed a federated data model, with hospitals managing the data, emphasising the need to involve hospitals in these reforms.

Multidisciplinary teams

Experts discussed using existing GP surgeries and health centres as a base for developing broader multidisciplinary teams. They shared international examples of how multidisciplinary teams can evolve to include salaried GPs and community health workers. Experts highlighted that integrating salaried GPs into these teams and using collective planning to distribute workload can increase job satisfaction and retention, especially for younger doctors. Examples from Brazil, Canada and Chile were mentioned.

Community health workers

Some experts advocated for formal mechanisms to integrate community health and wellbeing workers into the health and social care system. They explained that community health workers play a crucial role in managing chronic conditions, conducting community outreach and addressing social determinants of health, ensuring take-up of screening, improving treatment adherence. The Brazilian Family Health strategy, which uses community health workers, was discussed in detail. This model has already been piloted in the UK.
