Use this form to supply a statement of truth for first registration of land where the deeds are lost or destroyed.

Remember to include evidence of possession with your application. For a property this might be a recent utility or council tax bill or evidence of receipt of the rent and profits. For land, it may be a statutory declaration or statement of truth detailing how the applicant is in possession of the land.
