Fill in this form to pre-notify the Animal and Plant Health Agency (APHA) of your import of POAO if both of the following apply:

• safeguard measures are in place because of a new or emerging disease
• your commodity code is unavailable in the Import of Products, Animals, Food and Feed System (IPAFFS).
