Details

To verify your child for the childcare service, you will need to use the Child Verification form (CVF1).

How to use the print and post form

You will fill this form in online and you cannot save your progress. It is best to get all of your information together before you start.

If you’re using an older browser, you’ll need to update it or use a different browser. You can find out more about browsers.

Documents you need to send with the form

You will need to send one of the following documents for your child:

  • passport
  • birth certificate
  • adoption certificate

Do not send photocopies.
