This content was originally produced by Public Health England (PHE). This content is now owned and managed by the UK Health Security Agency (UKHSA).

This toolkit supports UKHSA teams investigating incidents where an infection control breach or failure has led to potential risk of BBV transmission.

The toolkit supplements incident response. Teams should investigate incidents using standard procedures, and follow existing reporting and escalation arrangements such as, the National Incident and Emergency Response Plan (NIERP).
