Before you start work

Before you start any alteration work you need to contact us through the details below.

You should then complete and send us the permission application form, with:

  • your medical evidence – for example, a letter from your GP
  • evidence of how you’ll pay for the alterations

If you do not send all the information we need, your application may be delayed or your request refused.

After you’ve applied for permission

After you’ve sent us the forms, you’ll need to pay an administration fee.

When the work is completed

When your alterations are completed you need to send us the ‘declaration of work completed’ form.
