You need to complete the part A form for most environmental permit applications. That includes if you are applying:

• for a new permit
• to vary (change) an existing permit
• to surrender (cancel) your permit
• to transfer an existing permit to yourself

1. 4 November 2025

   Updated the form document. Added a new introductory page, including a list of when to use the form. Added a new section 8 for environmental record checks. Added more guidance throughout.

2. 17 March 2025

   Removed reference to flood risk activities from question 3.3 guidance.

3. 28 February 2025

   Updated summary and details as this form must now be used for surrender applications as well. Updated the form in a few ways. Added definitions and guidance to clarify questions. Added new category of applicant - government department. Added new contact details section for discussing the application, to receive official documents and operational contact details. Added new section for outgoing operator details. Updated where to send applications to include flood risk activities. Removed separate guidance document for the form.

4. 22 October 2020

   Updated application form to delete old ref to F2 form (no longer used). Updated guidance to Part A to update postal and email addresses.

5. 13 February 2020

   Application form Part A - information about you: Added additional text to clarify that date of birth information is only needed when applying for a new or to transfer an installation or a waste operation.

6. 22 January 2020

   We have removed the environmental permit application privacy notice. This information is now in Part F1: apply for an environmental permit (Opra, charges, declarations) application form.

   We've added our environmental permit privacy notice.

8. 3 April 2018

   This page has been updated by the Environment Agency as a result of the strategic review of charges. The review was consulted on between November 2017 and January 2018. The changes commence from April 2018.
