A form for reporting outbreaks of acute respiratory infections in institutions, which does not require virological confirmation.

This has been withdrawn because of changes to the outbreak reporting practice, with no longer a need for local teams to use this form to report every acute respiratory outbreak to the national surveillance team. Instead, the outbreak information will be gathered from the HPZone system inputted by the local teams for national surveillance purpose.

If an outbreak is ongoing please use this form to notify the flu section of the Respiratory Diseases Department at PHE Colindale.

Send final form with all data as soon as possible after the outbreak is over.
