Significant changes to an academy: tier 1 application form

Use this form to apply to make tier 1 significant changes to an academy.

Who can use this form

Only academy trusts should complete this form.

Local authorities should not use this form, even if the change is being proposed at their request.

What this form can be used for

This form should only be used to apply for tier 1 changes (single or multiple) to one academy.

To make changes to multiple academies, you need to submit a form for each individual academy. The same consultation summary may be used for each form if a joint consultation has been conducted.

What this form should not be used for

 You should not use this form to apply for tier 3 changes. These are:

  • amalgamations
  • de-amalgamations
  • gaining, changing or removing a faith designation  
  • changing a Church of England academy’s characteristics
  • any change to a University Technical College (UTC)

Before you start

This form has 4 sections:

  • Section 1 – screening questions 
  • Section 2 – details of the changes
  • Section 3 – personal information
  • Section 4 – Public Sector Equality Duty

All sections must be completed. The form cannot be saved, so before you start you should:  

Start your application on tier 1 application form.

Your personal information

Information provided in this application form, including personal information, may be subject to publication or disclosure in accordance with the Freedom of Information Act 2000, Environmental Information Regulations 2004 or the Data Protection Act 1998. The Department for Education will process your personal data (name and address and any other identifying material) in accordance with the Data Protection Act 2018, and your personal information will be used to process your significant change application.

Additional evidence or information

Further evidence relevant to your application should be emailed to significantchange.academies@education.gov.uk.

Email the evidence as soon as possible, following the submission of your application form, and include the academy name and unique reference number (URN) in the subject line.

Any delays in emailing it may mean it takes us longer to process your application.
