This memo is for Work and Health Programme (WHP) providers.
Background
1. This memo informs you of amendments that have been made to Work and Health Programme including JETS Provider Guidance WHP07 form in Annex 2 Forms and letters which are held in Jaggaer.
2. A new part 4 has been added to the WHP07 Change of Circumstances notification form to outline steps required by Jobcentre Plus when a Participant disengages from the programme the summary of which is included in the table below.

3. The new version of the WHP07 change of Circumstances notification form version 02/2022 can be downloaded from Jaggaer and should be used with immediate effect by you and your supply chain partners.
