Claim forms and guidance for people who may be able to claim compensation through the Windrush Compensation Scheme.

  1. Updated the Primary claimant, Representative of an estate, and Close family member forms and guidance to change the term 'maiden name' to 'name before marriage'.

  2. Updated the primary claimant, representative of an estate and close family member guidance and corresponding forms. A list of changes can be found in each guide.

  3. Updated the guidance on Requesting that a claim be considered out of order to show that priority will be given to people aged 75 and older.

  4. Forms and guidance updated to align with the launch of the Windrush Compensation Advocacy Support Fund.

  5. Forms and guidance updated to reflect the transition from biometric residence permits to eVisas.

  6. Updated guidance to include information on using eVisas as proof of identity.

  7. Amended the following guidance to reflect a change in operational processes for claim allocation order and the decision making process: Close family member: Windrush compensation claim guidance, Requesting that a claim be considered out of order, Primary claimant: Windrush compensation claim guidance and Representative of an estate: Windrush compensation claim guidance.

  8. Correction to the dates in the close family member guidance, section 1, option (f). "This means the latest date they were in the UK was 31 December 1972" - not 31 December 1973.

  9. Primary claimant and Close family member guidance and form have been updated to clarify medical evidence in support of a claim. Representative of an estate guidance and form have been updated with details of where to find information about the support available.

  10. Added information on what you need to do if you fill in the form on a computer.

  11. Changes to the eligibility criteria, and updated company name for We Are Group.

  12. Information on considering a claim out of order updated.

  13. Update made to Representative of an estate: Windrush compensation claim guidance, Close family member: Windrush compensation claim guidance and Primary claimant: Windrush compensation claim guidance.

  14. Close family member claim form updated.

  15. PDF versions of guidance added for primary claimant, close family member and representative of an estate.

  16. Form and guidance updated for primary claimant, close family member and deceased estates to reflect changes to the scheme.

  17. Updated guidance on claims by: primary claimant; close family members; representative of an estate.

Details

Use these forms if you are either claiming compensation:

  • for yourself
  • if you are a close family member
  • if you represent someone’s estate

How to claim depends on who you are and if you are in the UK or overseas.

You should read the relevant form guidance before you apply.

If you’re filling in the form using a computer or mobile device

You need to:

  • save a copy of the form before entering your information
  • type directly into the form - do not paste text into the box from another document

Pasting text into the box might mean some of the text is not displayed correctly when you send your form.

If you need more space, add the additional information to a separate document.
