Attached is a copy of the adverse event report for your dog that was received by VMD from the Marketing Authorisation Holder.
The sender report identification case reference number included in the report document is the internal reference number for this case in the VMD database, this may be different to the worldwide case number and/or Marketing Authorisation Holder’s internal reference number.
Under the Freedom of Information Act 2000 we are required to protect personal data in line with the UK GDPR and the Data Protection Act 2018. This means that names, addresses, phone numbers, and any other identifiers relating to individuals have been removed from the documents provided. The purpose of this redaction is to safeguard privacy and prevent the disclosure of information that could identify a living individual, unless there is a lawful basis for disclosure.
The only other information we hold about you is any email correspondence you have sent to us and the responses we provided to you. We do not hold any additional records beyond the adverse event report supplied.
