This document was reviewed and updated in April 2011. The guidance is updated every 3 years and antibiotic guidance every 2 years; or more frequently if there are significant developments in the field.

You can make minor changes to the Microsoft Word version to suit local service delivery and sampling protocols. But we would discourage major changes to the guidance.
