Details

Use this form to apply for a licence to move animals from your restricted farm or premises if the restricted destination farm or premises cannot apply. 

Completed application forms should be sent to the Animal and Plant Health Agency (APHA) at: CSC.TBlicensing@apha.gov.uk 

Applications can take up to 5 working days to process. It may take longer if veterinary checks are needed or if we need more information. 

If you need help, call 03000 200 301 and select the option for TB in animals. Lines are open Monday to Friday, 8:30am to 5pm.
