Safety Bulletin 26 – CO2 fire suppression installation testing

Applicable to all vessels with fixed carbon dioxide (CO2) fire suppression systems installed.

Details

Following the failure in activation of a CO2 fire suppression system on a roll-on/roll-off (ro-ro) cargo ship, it has been discovered that the pilot lines were both blocked and leaking. It has been determined that was caused by a faulty hose coupling. The CO2 system failed to activate using the remote actuation mechanism and the crew were not able to operate the manual override.
