You should only use this form if the PSC has already been given 2 extensions using the ‘Provide identity verification details for a person with significant control’ service (opens in new tab).
If you need to request an additional extension, use this form to submit your request before the PSC’s current verification deadline has passed. You’ll need to upload supporting evidence.
After you submit your request, an examiner will consider the information and evidence you provide.
If your request is accepted, the PSC’s deadline will be extended by 14 days. They must comply with identity verification requirements by their new deadline. If the request is rejected, they must comply by their current deadline.
If the PSC does not comply on time, they’ll be breaking the law and we will send them a default letter setting out the offences relating to identity verification.
