Only use this form if you’ve already been given 2 extensions using the ‘Provide identity verification details for a person with significant control’ service.
If you need to request another extension, use this form to submit your request before your current verification deadline has passed. You’ll need to upload supporting evidence.
After you submit your request, an examiner will consider the information and evidence you provide.
If your request is accepted, your deadline will be extended by up to 14 days. You must comply with identity verification requirements by your new deadline. If the request is rejected, you must comply by your current deadline.
If you do not comply on time, you’ll be breaking the law and we will send you a default letter setting out the offences relating to identity verification.
