Predominantly used for 4 and 5 star, and major chain operated, hotels requesting information relating to accounts.

Why your information is important

The basis of rateable value is the annual rent for a property as if it was available on the open market at a fixed valuation date. The information you provide on this form is used to help ensure that the assessment of rateable values is correct. Local authorities use rateable values to calculate business rate bills.

when issued you must return this form within 56 days from the day you receive it. If you do not return this form within 56 days you will be liable to a penalty of £100 see paragraph 5A(1) of Schedule 9 to the Local Government Finance Act 1988 (“the Act”).

Before you start

This form is interactive (one that you complete on screen) and you must use Adobe Reader to complete it. Downloading Adobe Reader is free and versions are available for all computers and mobile devices. Search on your internet browser for Adobe Reader or download from the App Store or Google Play Store.

To make sure the form will download or open in Adobe Reader:

  • Windows users should right click on the form link then select ‘Save target as’ or ‘Save link as’
  • Mac users should right click on the form link then select ‘Save linked file as’
  • Mobile device users should download the form and save it to your device. Open the Adobe Reader app and browse to and open the file.

Once complete, save the form and attach the completed form to an email to specialist.rating@voa.gov.uk with the subject heading “Completed FOR xxxxxxxx/xxx (the 11 digit reference number from your letter)” and submit.

You can also choose to print and complete the form by hand.
