Before you complete this form, please read the Explanatory Leaflet (PMR 1) on the EC Poultrymeat Marketing Regulations: Special Marketing Terms.

Producers and processors that use or are planning to use the Special Marketing Terms (SMT) for poultrymeat are required to register their premises with Animal Health and Veterinary Laboratories Agency (AHVLA) and the Food Standards Agency, respectively.
