Use this form if you are in the UK with a visa for Tier 1, 2, 4 or 5 and you need to notify UK Visas and Immigration (UKVI) of a change in your circumstances.

This includes changes to:

• personal details
• criminal convictions
• contact details
• representative’s details
• dependants’ details
• any other information you think UKVI should know
