Infected blood interim estates payments: compensation application form where a company is the personal representative
Apply for interim compensation as a registered professional who has been appointed by a company who is the executor of the estate of someone who has died after receiving infected blood.
Documents
Details
Compensation will be paid to the estates of deceased infected beneficiaries who were registered with an Infected Blood Support Scheme (IBSS) or an Alliance House Organisation Scheme on or before 17 April 2024. Check if you’re eligible to apply.
Use this form to apply for an interim compensation payment of up to £310,000 if this is your first time applying for an estates interim payment, or probate has changed since your previous application, or if you wish payment to be made to a different recipient from your previous interim payment.
If the estate has already received £100,000 in interim payments, the same personal representative is applying, probate remains unchanged since the previous application to an IBSS, and payment is to be made to the same recipient as the previous interim payment, there is an expedited application form that requires less information than provided previously. Contact the IBSS you are applying to for more information.
If you need a printed or accessible version of the form, contact the IBSS in the nation where the person who died was infected.
Before you start
You’ll need a certified copy of one of the following probate/confirmation documents which names the company as the executor of the deceased infected person’s estate:
- a grant of probate
- a grant of confirmation or confirmation ad omissa/ad non executa
- letters of administration
- chain of representation
- grant de bonis non
You’ll also need to include:
- a formal letter from the company executor appointing you as the registered professional to act on behalf of the estate (this must be on company letterhead, signed by an employee of the company and include their position at the company)
- information about you, the appointed professional - professional registration details about you and the registered firm you work for
- information about the company - registered name, address, registration number and Unique Taxpayer Reference
Chains of representation
Applications supported by a valid chain of representation will be accepted. If you have a chain of representation, and are applying for compensation read our guidance on how to apply for compensation with a chain of representation.
If you do not have probate, or if you’re not sure if it’s already been granted, find out about applying for probate or confirmation. You might be able to claim back the costs of applying for probate.
Grant de bonis non
Applications supported by a certified copy of a grant de bonis non will be accepted. If you have a grant de bonis non, and are applying for compensation read our guidance on how to apply for compensation with a grant de bonis non.
Where to send the form
Send your compensation application form and certified copies of your supporting documents to the IBSS in the part of the UK where the person who died was infected.
