Details

In a January 2008 Gordon Brown committed DH to bring forward a Patients’ Prospectus during 2008. This sets out how we will extend access to a choice of self care services to all 15.4 million people with a long term condition. This commitment provides an opportunity to draw together all the existing strands of work/information in terms of support for self care and put them into a format that is accessible and understandable for patients - putting a public face on existing policies. ‘Your Health, Your way - a guide to long term conditions and self care’ was launched on NHS Choices in November 2008.
