Awarding organisation Issue Action taken Case status
Innovate Awarding Ltd (IAO) Failure to comply with Condition A5 - Availability of adequate resources and arrangements; Condition C1 - Arrangements with third parties On 23 May 2018 Ofqual accepted an undertaking from IAO. Closed on 25 October 2018

Following an Ofqual audit programme which reviewed the arrangements put in place by awarding organisations for monitoring centres delivering regulated qualifications, Ofqual identified defects in IAO’s monitoring arrangements which are vital to ensure the delivery and award of valid qualifications. Innovate Awarding Ltd (IAO) accepted that it had breached our General Conditions of Recognition and gave Ofqual an Undertaking to secure compliance with the Conditions.

Following the audit, IAO has taken prompt action to introduce a system of control to ensure that Lead External Quality Assurance (EQA) reviews and feedback are fully documented and has amended its systems of control to ensure that qualification certificates of centre staff are checked and documented by EQAs. IAO has undertaken to carry out documented observations of all active IAO EQAs by September 2018 and establish an ongoing annual program of documented EQA observations.

In line with general condition B8, IAO must now take all the steps identified in the undertaking. We may take further action if it doesn’t follow through on its commitments.
