You can use these forms to:

  • apply for a firearm or shotgun licence
  • renew a firearm or shotgun licence
  • vary a firearm certificate

Application guidance notes can be found at the end of form 201.

The form to be completed by the doctor is included in the application forms.

  1. Welsh language versions of the forms have been added.

  2. Added note to explain that the form to be completed by the doctor is included in the application forms.

  3. Added final versions of form 201, form 201V and form 116, for use from 1 November 2021.

  4. Added draft versions of form 201, form 201V and form 116. These revised application forms will come into effect from 1 November 2021.

  5. Updated forms for use from 31 December 2017.
