How to complete a child deed poll exhibit cover sheet

Before you start
Before applying you should first read the full guide on changing your or a child’s name by deed poll. This includes:

If the child was born in Scotland, you should follow the rules and guidance for changing your name in Scotland.
You must also complete:
- a child deed poll form if applying by post (not needed for online applications)
- an affidavit (statement) of best interest
- a deed poll statutory declaration for a child
- photocopies of certain documents – these will be your ‘exhibits’. See the full guide for information about the documents you must include
If you apply online, you do not need to download the cover sheet. It will be emailed to you to print and complete after you have applied.
Complete a cover sheet for each exhibit
An ‘exhibit’ is a document used in court as evidence. The court needs exhibits as proof of who your application says you are.
In the statutory declaration the exhibits are referred to by letters:
- Exhibit A – a copy of the deed poll form (LOC022) – you only need to complete this if applying by post
- Exhibit B – a photocopy of the child’s full birth certificate as evidence that they are a British, Commonwealth or British dependent territories citizen
Include a cover sheet for each exhibit with the relevant exhibit wording and signed by the same solicitor, commissioner for oaths or officer of the court who witnessed the declarant make their oath or affirmation.
The exhibit sheets must include the following wording:
“This is the exhibit marked ‘A’/‘B’/‘C’ referred to in the declaration of [enter statutory declarant’s name (the person who has known you for 10 or more years)] declared before me [enter name of authorised witness] this [day] day of [month] in the year [year].”
You can print and use the template cover sheet. Alternatively, you can use a blank piece of paper if it includes the required wording and is signed by the solicitor, commissioner for oaths or officer of the court.
