Ordering forms if you are a member of the public

If you are a member of the public and you need a DWP leaflet or claim form, you need to contact your local Jobcentre Plus.

For medical forms, (for example, a Med3 or SR1) contact your doctor or healthcare professional.

If you need a MATB1 to claim Statutory Maternity Pay or Maternity Allowance, speak to your doctor, midwife, or a healthcare professional. MATB1s cannot be ordered by healthcare professionals outside of the UK, unless they are British Armed Forces staff.

Any requests from members of the public via this channel for leaflets or medical forms will not be acknowledged. Find out about benefits and financial support.

Advisors and intermediaries

If you do not work for Department for Work and Pensions (DWP) but wish to order DWP claim forms, reply envelopes and information materials, please complete the order form for DWP claim forms and information materials.

If you have permission to send a form, you must submit it electronically and email it to dwp@hhglobal.com.
If you submit the form by fax, handwritten or as a paper copy, it will not be processed.
