How to order DWP medical evidence forms: guidance for healthcare professionals

All pre-printed Med 3, Med 10, MATB1, SR1 and PIP1SREL pads must be always kept securely. Any outdated or damaged versions must be destroyed immediately via confidential waste.
Forms that can be ordered
Ordering MED 3 forms
To order pre-printed Med 3s, the healthcare professional or organisation should be delivering NHS Services and treating NHS-funded patients.
All NHS GP practices can now produce a Med 3 electronically (in English and Welsh). Please check that this facility is being used before placing an order for a pre-printed form.
You can only order a maximum of 25 pre-printed Med 3 pads. We have limited the numbers per order, as these can now be produced electronically within GP practices. In a hospital setting, electronic Med3s will be implemented shortly. For the latest information please see: eMED3 (fit notes) in Secondary Care - NHS Digital.
A product code is not needed. Please allow up to 7 working days from receipt of order to delivery.
Private practice and private hospital
Healthcare professionals working in private practice or private hospital that are not treating NHS funded patients do not have access to the pre-printed Med 3.
You should speak to your employer about your local policy relating to the issue of Med 3 forms for private patients.
Ordering MED 10 forms
Pre-printed Med 10s are issued in a hospital setting to cover any period that a patient is in hospital. Pre-printed Med 10s are available in pads of 25.
Ordering MATB1 forms
The MATB1, contains a unique certificate number, and is available to both private and NHS practices and hospitals. MATB1 is available in pads of 20.
Ordering SR1 and PIP1SREL forms for end of life
Healthcare professionals, doctors, hospital doctor and registered nurses (for example, specialist nurse) can complete form SR1 and PIP1SREL (PIP1SREL for use by Macmillan only).
How to place an order
England, Scotland and Wales
If you are a healthcare professional and need to order pre-printed forms, you can place an order by:
1. Downloading the order form you require
2. Email your completed order form for the Med 3, Med 10 and MatB1 to this DWP inbox: MA.order@dwp.gov.uk.
3. Orders for the SR1 and PIP1SREL forms for end of life should be emailed to dwp@hhglobal.com.
4. Please allow up to 7 working days from receipt of your order to delivery.
